We also
provide services for elders experiencing mental health issues such as
depression, grief, family conflict, and difficulty in coping or
dementia.
Identification of At Risk Elderly in the
Community
– cooperating with community partners to identify elderly people at
risk of deterioration or hospitalization.
Case Management
– planned, coordinated treatment and assistance in securing services and
entitlements. Individual and family counseling as needed.
Outreach to Isolated Elderly
– in-home assessment and case management supporting independent living.
Consultation and Treatment in Nursing Homes
– on-site mental health assessment, consultation and brief treatment in
long term care facilities.
